Mode of Payment (DD / Cash):
On Cash payment: From only The State Bank of India, Jipmer campus branch from 25.02.2009 to 15.04.2009.
Through DD: By a M.I.C.R. Bank Draft of Rs.750/- (UR/INST/OBC) and Rs.500/- (SC/ST) drawn in favour of "Accounts Officer, Jipmer" payable at any Nationalilzed Bank in Puducherry, Union Territory. Two 13 cm width x 7 cm height self addressed labels must accompany the postal request for the correct delivery of the Application Forms.
Application Forms:

• In Person: Candidates can be obtained on cash payment of Rs.750/- (for Unreserved UR/Institute INST/Other Backward Classes OBC) and Rs.500/- (SC/ST) from only The State Bank of India, Jipmer campus branch from 25.02.2009 to 15.04.2009.
  
  
• By Post: Application forms can also be had from professor (Academic), Jipmer, Puducherry - 605 006 up to 8th April 2009 through postal requests, accompanied by a M.I.C.R. Bank Draft of Rs. 750/- (UR/INST/OBC) and Rs.500/- (SC/ST) drawn in favour of "Accounts Officer, Jipmer" payable at any Nationalilzed Bank in Puducherry, Union Territory.
  
  
• Through Online: Application forms can be submitted by an On-line process (Internet based) by accessing the website: www.jipmer.edu.in from 25.02.2009 to 08.04.2009. The candidate should first pay the non-refundable application fee of Rs.750/- (UR/INST/OBC) and Rs.500/- (SC/ST) by obtaining a demand draft as mentioned above. The draft details are required while filling up on-line form. Candidates are required to take a print out of the online application form with the number after successful submission of data. The printed Form with candidate's signature, left thumb print, photograph and the DD must be sent by Speed Post so as to reach the Professor (Academic), Jipmer, Puducherry - 605 006 by 15th April, 2009 - 04.00 PM.
  
  

Note:

• On-line applications will be issued Hall-Ticket for Entrance Examination only after the postal receipt of the completed form, Bank Draft and necessary enclosures by the Office of Professor (Academic), Jipmer. Unsigned/Incompletely filled in applications/Applications received after 15th April 2009 will be rejected and drafts enclosed will not be refunded. No further correspondence will be entertained in this regard.
  

The completed applications should reach the examination section of the institute by 04.00 PM on 15.04.2009. The admission notice is available on www.jipmer.edu.in.
